Avalon Health Clinic – Patients Module

Overview
This module manages patient records, including adding, searching, and editing patient details.
Step-by-Step Guide
1. Adding a New Patient
Click "Add Patient." on the sidebar, Fill in required details which include:
The doctor fills in essential details to ensure accurate record-keeping and streamlined medical care. The required fields include the patient’s prefix, first name, full name, middle name (optional), gender, date of birth, and primary phone number. Additional optional fields include national ID, passport number, nationality, primary and secondary email addresses, secondary phone number, full residential address, and country. If the patient has known allergies, the doctor can also record them. Insurance verification is available for patients with medical aid, where the doctor enters the insurance provider, member number, and dependent code before verifying the details. The system also allows the doctor to input next of kin information, which mirrors the patient’s contact details with an additional field for specifying the relationship. If applicable, the doctor can enter referral information, including referral type and referral email. After completing all necessary fields, the doctor submits the form to successfully register the patient in the system.

After filling up the required details of the patient the doctor can click on Submit Patient button on the bottom right
2. Searching Patient
Use the search bar or the Search Saved Patientslink on the sidebar and enter the patient’s name and surname, file number of the Organisatin or ID. Click Search

3. Editing Patient Details
After clicking Search the patients documents will be displayed. If you wish to edit the information, click on the Edit button. After editing Submit the changes and the information is updated

4. Requesting Patient Access
Enter the patient’s Avalon ID, the period you are requesting the files (e.g 1hour, 1day, 1week, 1month, 1year) and request access.
